This package is built around partnership. We take an active role in monitoring circumstances, coordinating services, and supporting decision-making—ensuring that emerging needs are identified early and managed thoughtfully.

Support includes weekly check-ins, ongoing coordination of services, advocacy with health professionals, and assistance preparing for assessments such as My Aged Care or NDIS. We also provide regular communication with families (with consent), helping to reduce uncertainty and strengthen confidence.
