You’ll have a dedicated point of contact who understands your situation and can help you navigate services, while also keeping an eye on any changes that may need attention.
Support includes a monthly wellbeing check-in, along with ongoing phone and email support between contacts. We can assist with navigating My Aged Care, NDIS, and community services, as well as connecting you with appropriate local supports.
Those seeking occasional guidance, reassurance, and the comfort of knowing someone is there if needed.
